Abstract The aim of the study is to report the outcomes of endovascular management of congenital portosystemic shunts (CPSS) in three patients. This is a retrospective review of three patients who underwent transcatheter shunt closure for CPSS between 2023 and 2025. Pre- and post-procedure imaging, procedural details, and clinical follow-up were analyzed. Successful shunt closure was achieved in all three cases with significant improvement in hyperammonemia, pulmonary arterial hypertension, and hepatopulmonary syndrome. There were no procedure-related complications. Endovascular closure of CPSS is safe and effective in selected patients, offering good outcomes and low morbidity when guided by precise imaging and hemodynamic evaluation.
