ABSTRACT Turner syndrome (TS) leads to various abnormalities, including renal malformations that can progress to end‐stage renal disease (ESRD). Although rarely performed in patients with TS, renal transplantation offers significant benefits over dialysis. We report the case of a 34‐year‐old female with TS and ESRD who underwent a successful living‐donor kidney transplantation. Preoperative imaging revealed atrophic, malformed native kidneys located entirely in the pelvic region. A suitable graft bed was meticulously created in the right iliac fossa without the need for native nephrectomy. At the 9‐month postoperative follow‐up, the patient demonstrated excellent and stable graft function. This case highlights the feasibility of kidney transplantation in patients with TS and underscores the importance of careful preoperative planning and a multidisciplinary approach to overcome unique anatomical challenges.
