We present the Continuity Index Quantum (CIQ) framework — a unified field theory that derives gravitational structure formation, dark matter halos, cosmological expansion, and neurophysical consciousness from a single informational master equation. Gravity is recast as a contrast operator L2 that selectively concentrates quantum field fluctuations (L3) into standing-wave condensates (L5, matter). A five-layer causal hierarchy (L1–L5) coupled to entropy (S) and decoherence (E) fields produces: (i) BAO wiggles matching observational data, (ii) a CI-Hazard rate h (u) reproducing the Friedmann expansion with mean residual 0. 0255% and local Hubble rate H (z=0. 01) = 67. 88 km/s/Mpc (vs. 68. 29 in Lambda-CDM), (iii) dark matter as a thermodynamic condensate at phase boundaries (SigmaSDM = 131. 70), and (iv) stability quotient Q ≈ 15, 686. 9 from vacuum fluctuations (Q0 = 2. 04). The RSH dynamical system maps the CIQ layers one-to-one onto neurophysical observables (gamma synchronisation, prediction error, synaptic memory), yielding a quantitative theory of consciousness as the local collapse of quantum correlation fields by condensed neural structure.
