Stakeholder feedback plays a central role in program evaluation, offering insights into effectiveness, satisfaction, and opportunities for improvement. This article presents a structured template for collecting stakeholder input through a mix of quantitative ratings and open-ended qualitative responses. Designed for diverse non-profit contexts, including low-resource environments, the framework guides organizations in gathering, analyzing, and applying feedback to enhance program delivery and accountability. Illustrated with a community literacy case study, the paper demonstrates how structured feedback mechanisms improve program design, foster stakeholder engagement, and support continuous learning and improvement.
