Collecting sexual orientation and gender identity (SOGI) data in transgender dermatology research creates tension between protecting patient privacy and ensuring equitable study inclusion. In our scenario, inviting a transmasculine adolescent to participate during the visit risks involuntary disclosure to a parent, while withholding the opportunity reinforces underrepresentation. A secure patient portal offers a safer, autonomy-supporting approach and illustrates the need for transparent data practices and patient centered recruitment in TGD research.
