Filariasis is a public health problem that is included in the Neglected Tropical Diseases (NTDs). Until 2020, Pekalongan City, Indonesia, has carried out two rounds of Mass Drug Administration (MDA) to reduce the infection rate (mf rate) below 1%. This study aime to detect new infections and transmissions of filariasis in Pekalongan City, Indonesia, using a cross-sectional design conducted from March to August 2021. A total of 100 venous blood samples, taken from respondents who came voluntarily, were used as samples for antigen examination using a Rapid Diagnostic Test (RDT).As many as 7% of the blood samples examined showed positive antigen, with 57.14% found in men and 42.86% in women. This condition illustrates that the incidence of filariasis remains relatively high (>1%). This study found that the incidence of filariasis infection was 7% and indicated the existence of transmission in Pekalongan City.
