Retinoblastoma, the most common intraocular malignancy in children, usually presents with leukocoria or strabismus. In later stages, the most frequent sign may be proptosis of the eye. This neoplasm can mimic any orbital or ocular disease in its atypical clinical or radiological form. Tuberculosis is a highly infectious disease. Presentation of tuberculosis can mimic any disease and may lead to misdiagnosis of this treatable condition. Here, we report a case of disseminated tuberculosis with intraocular involvement, initially diagnosed as retinoblastoma, but later identified as ocular tuberculosis. The infant responded well to anti-tubercular therapy with resolution of the intraocular disease.
