We report a rare case of anterior capsular contraction syndrome (ACCS) presenting as a figure-of-eight-shaped capsular opening due to fibrotic bridging across the capsulorhexis in an Indian patient. The condition developed more than a decade after uncomplicated phacoemulsification and was documented with slit-lamp imaging. Despite the unusual morphology, the intraocular lens remained stable, and visual acuity was unaffected. To our knowledge, this represents one of the first reported cases of fibrotic bridging leading to figure-of-eight morphology in the Indian population. Long-term postoperative surveillance remains critical to recognize and manage such rare presentations.
