Nanophthalmos is a relatively rare bilateral ocular developmental disorder characterized by structurally normal but small eyes. This disorder presents with small corneal diameter, shallow anterior chamber, narrow angles, high lens/eye volume ratio, uveal effusion, and a high risk of complications following intraocular surgeries. We highlight a series of secondary angle closure due to uveal effusion in three nanophthalmic eyes in this report. This is an uncommon but visually significant pathology that needs meticulous workup and management.
