Abstract Background Glucagon-like peptide-1 receptor agonists are novel anti-hyperglycemic agents with proven cardiovascular benefits. While semaglutide may reduce atrial fibrillation incidence, the effect of tirzepatide remains uncertain. Methods PubMed, Embase, Cochrane Library, and ClinicalTrials.gov were searched for randomized controlled trials (RCTs) comparing tirzepatide vs. placebo for patients with overweight and obesity. Risk ratios (RR) with 95% confidence intervals (CI) were pooled with a random-effects model. Results Our meta-analysis included 9 RCTs including 6,290 patients, of whom 4,341 (69%) were randomized to receive tirzepatide. There was no significant difference between tirzepatide and placebo in all arrhythmias (1.15% vs. 1.28%; RR 1.49; 95% CI 0.94-2.37; p=0.093; Figure 1A), atrial fibrillation (0.79% vs. 0.83%; RR 1.82; 95% CI 0.96-3.45; p=0.067; Figure 1B), and all atrial arrhythmias (0.79% vs. 0.78%; RR 1.69; 95% CI 0.93-3.06; p=0.08). Conclusions In patients with overweight or obesity, tirzepatide had no significant effect in atrial fibrillation incidence compared with placebo.
