This work proposes the Control-State Gate Hypothesis of metastatic cancer, suggesting that lethal cancer progression may require a prior systems-level transition in organismal physiological regulation rather than arising solely from cumulative cellular mutation. The framework predicts that metastasis-permissive states emerge from distributed neural and neuro-immune regulatory remodeling, reduced adaptive physiological flexibility, and altered system-level control dynamics.
