Abstract Background: Tympanoplasty is a common surgical procedure for repairing tympanic membrane (TM) perforations. Platelet-rich plasma (PRP) is hypothesized to enhance graft uptake and hearing outcomes due to its regenerative properties. Objective: To evaluate the efficacy of PRP as a reinforcement agent in type 1 tympanoplasty for improving graft uptake and hearing outcomes in large perforation of the TM. Study Design and Materials and Methods: This randomized controlled trial included 70 patients with chronic otitis media (COM) mucosal type and large central TM perforations. Participants were randomized into two groups: Group A underwent tympanoplasty with PRP and Group B underwent tympanoplasty without PRP. Outcomes assessed included graft uptake and hearing improvement, measured using pure tone audiometry over a 3-month follow-up period. Results: Graft uptake was successful in 91.4% of patients in Group A and 88.6% in Group B ( P = 0.69). Mean hearing improvement was 8.35 ± 2.83 dB in Group A and 8.71 ± 3.20 dB in Group B ( P = 0.31). Both groups showed statistically significant hearing improvement compared to preoperative levels, but no significant intergroup difference was observed. Conclusion: While PRP demonstrated excellent safety and feasibility in tympanoplasty, it did not result in statistically significant improvements in graft uptake or hearing outcomes compared to conventional tympanoplasty.
