Abstract We report two cases of lumbar osteochondromas treated with minimally invasive transforaminal lumbar interbody fusion (MI-TLIF). Both patients presented with low back pain and radiculopathy, and underwent MI-TLIF with complete excision of the bony outgrowths. The lesions appeared hypointense in Case 1 and hyperintense in Case 2 on magnetic resonance imaging. Histopathological examination confirmed osteochondroma. Both patients experienced relief of symptoms postoperatively, with improved mobility and quality of life. MI-TLIF can be an effective surgical intervention for lumbar osteochondromas, with favorable outcomes observed in these cases. Long-term follow-up is essential to monitor for recurrence or malignant transformation. These cases contribute to the existing literature on lumbar osteochondromas and highlight the importance of considering this condition in the differential diagnosis of lumbar spine pathologies.
