The review discusses the relationship between the mechanisms of migraine pathogenesis, namely, cortical spreading depression, changes in cerebral circulation during a migraine attack and with cortical spreading depression, and the role of serotonin and the calcitonin gene-related peptide in regulating cerebral vessels and their involvement in the emergence of a migraine attack. Drugs used for the treatment and prevention of migraine, as well as new compounds being developed for the treatment and prevention of migraine attacks, are described.
