Abstract Background and Aims: Vasopressin is a commonly used drug in gynecological surgeries to reduce perioperative bleeding. The aim of the study is to observe the hemodynamic effects of intramyometrial vasopressin injection and its efficacy to reduce blood loss in gynecological surgeries. Methodology: The study included 125 patients. Type of anesthesia was decided by the concerned anesthesia team. After intramyometrial injection of vasopressin by the surgeon, blood pressure, heart rate, arterial oxygen saturation, electrocardiogram changes, end tidal CO 2 were recorded at serial intervals till 1 h of the patient transferred to postanesthesia care unit. Other details, such as concentration and amount of vasopressin, speed and number of injections, blood loss, details of blood transfusion, events such as pulmonary edema, cardiac arrest were also noted. Results: The median age in this study was 40 years with interquartile range (32.50–45.00). In our study, hypotension was observed in 8.8%, hypertension was observed in 13.6%, bradycardia in 23.2%, tachycardia in 9.6% of participants. None of them had experienced cardiac arrest, pulmonary edema, or myocardial infarction. 11.2% of participants were required blood transfusion. Conclusion: The benefits of vasopressin during gynecologic procedures must be carefully weighed against the side effects. Therefore, anesthesiologists and gynecologists should adhere to protocol while using vasopressin.
