Abstract This commentary on a retrospective study of patients with HER2 positive biliary tract cancer highlights the importance of reflex and validated HER2 testing criteria; the discordance of next generation sequencing and HER2 immunohistochemistry; the potential prognostic and predictive role of HER2; and the necessity to define HER2 therapy resistance mechanisms.
