Primary endometrial squamous cell carcinoma of the uterus is a rare neoplasm occurring predominantly in postmenopausal women, constituting <0.5% of endometrial carcinomas. 1 Little is known about its pathogenesis and genetic alterations; 2 and management guidelines are limited. 3 The tumour is known to behave aggressively, with a worse prognosis than endometrioid adenocarcinomas of the same stage. 3 Here we present the case of a 60-year-old female who presented with dull pelvic pain and was found to have a large heterogeneous uterine mass. We use this case to illustrate the diagnostic considerations and potential pitfalls of this rare entity.
