Near-infrared chemiluminescence (NIR CL) imaging is crucial for accurate diagnosis of wound sites, as it operates without external excitation while offering high resolution, deep-tissue penetration, and high selectivity. However, materials that can achieve NIR CL with high luminescence intensity and sensitivity while also having therapeutic effects are still scarce. Moreover, the healing of infected wounds is complicated by the dynamic demands of the wound microenvironment. Thus, there is a need for an intelligent theranostic system capable of precise and efficient RONS regulation. Thus, we integrated NIR CL imaging with a dual-responsive Fe/Mn bimetallic supramolecular porphyrin hydrogel, enabling a comprehensive diagnosis and “inside-out” therapy. Within this system, endogenous peroxynitrite (ONOO–) activates real-time in vivo NIR CL imaging for inflammation monitoring, while endogenous H2O2 triggers CO release from Mn2CO10 to modulate inflammatory factors. The addition of an Fe/Mn bimetallic enhances NIR CL intensity by nearly 10-fold and exhibits exceptional antibacterial efficacy (>99% eradication), pro-angiogenic function, and antioxidant capacity. In vivo evaluation confirmed real-time inflammation tracking and a 95.5% wound closure rate within 12 days, achieving visualized healing via ONOO–-activated imaging. This work provides deeper insight into the treatment of bacterially infected wounds.
