Abstract We report the case of a 54-year-old female with an 8-year history of rheumatoid arthritis, referred to the ophthalmology outpatient department by her rheumatologist to evaluate for signs of hydroxychloroquine (HCQ) toxicity. Baseline spectral-domain optical coherence tomography of the macula revealed subtle thinning of the outer nuclear layer and disruption of the ellipsoid zone in both eyes, with preservation of the central foveal region. Thinning of the outer retinal layers in the parafoveal area produced an ovoid appearance, consistent with early HCQ retinopathy. This case highlights the importance of meticulous review of patient history, appropriate screening, and timely detection of HCQ-induced retinal toxicity to prevent irreversible visual impairment.
