A 39-year-old man with end-stage renal disease on haemodialysis developed a progressively enlarging sacrococcygeal mass. Laboratory abnormalities and histopathology confirmed uremic tumoral calcinosis, and 99mTc-MDP SPECT/CT demonstrated intense uptake with an additional occult lesion. Medical therapy targeting calcium-phosphate imbalance resulted in clinical improvement.
