The superficial spread of cervical squamous cell carcinoma to the endometrium, fallopian tubes, and ovaries is exceptionally rare, while the extension of high-grade squamous intraepithelial lesions (HSIL) to the proximal reproductive tract is even more uncommon. The co-occurrence of uterine leukoplakia and squamous metaplasia of the fallopian tube epithelium has not been previously documented. This study details the clinical and histopathological features of 2 patients and synthesizes the relevant data from previously reported individuals. Although the superficial spread of cervical HSIL or squamous cell carcinoma is rare, it is easily recognizable and merits attention. Careful evaluation of surgical indications is warranted for patients undergoing only cervical conization or hysterectomy. Additionally, clinicians should be aware of the potential for these lesions in postmenopausal patients presenting with pyometra.
