Abstract Background Mastopexy without implants remains challenging, particularly in achieving adequate upper pole projection and sustained shape stability. The “Brazilian Breast Sculpting” technique integrates multiple autologous approaches—dermoglandular flap, intramuscular and subcutaneous fat grafting, muscular sling, and internal bra—into a standardized protocol. Objectives This preliminary study evaluates the feasibility, safety, and patient-reported outcomes of the Brazilian Breast Sculpting technique in patients seeking mastopexy without implants. Methods A prospective case series of 38 patients (mean age 44 years, BMI ≤28) who underwent mastopexy using the Brazilian Breast Sculpting technique between January 2023 and November 2024. The technique combines six standardized steps: liposuction, Liacyr flap creation, intramuscular fat grafting, muscular sling, internal bra, and subcutaneous fat grafting. Patient-reported outcomes were assessed with the Brazilian Portuguese version of the BREAST-Q Reduction/Mastopexy module at 12-month follow-up. Results All 38 patients completed the standardized six-step protocol with a minimum 12month follow-up. Postoperative morbidity was low, with wound dehiscence in 5 patients (13.2%) and suture extrusion in 7 patients (18.4%), all managed conservatively. At 12 months, mean BREAST-Q scores (0‒100) suggest high patient satisfaction, particularly in Satisfaction with Breasts and Physical Well-Being (Chest), supporting the feasibility and patient acceptance of this approach. Conclusions The Brazilian Breast Sculpting technique appears to offer a structured autologous approach to mastopexy without implants, with preliminary results suggesting favorable patient-reported outcomes and an acceptable safety profile.
