Intra-abdominal tuberculosis is still an important public health problem in developing countries. 69-year old female patient was admitted to hospital with complaints of abdominal swelling, fatigue and weight loss. The patient's physical examination were detected signs of abdominal distension and ascites. Axial T2-weighted MRI scans revealed of signs the mesenteric mass area, omental thickening and diffuse fluid accumulation in both the paracolic areas. The result of aspiration fluid cytology was negative for malignant cells. Histopathological examination of the biopsy showed described granulomatous nodules with epithelioid cells and Langerhans-type giant cells that are compatible with tuberculosis infection. After 6 months of antituberculosis treatment, the patient's clinical condition improved and the cancer antigen (CA-125) level returned to normal range. We present here a female patient with a marked increase in the level of CA-125, who is initially diagnosed with metastatic ovarian cancer and has common peritoneal masses and ascites.
