CONCLUSIONHypereosinophilic syndrome is often diagnosed late.There are diverse skin manifestations in hyper eosinophilic syndrome like, urticarial and angioedema, erythematous papules and plaques, erythrodermia, eczema like dermatitis, vesiculobullous lesions but necrotic ulcers as cutaneous vasculitis is not common finding.Given that this disease can cause involvement of various parts of the body, including the heart and kidneys, its rapid diagnosis and treatment are inevitable.The overlap of these symptoms with more common conditions can mislead healthcare providers, resulting in a focus on treating the manifestations rather than addressing the underlying eosinophilic disorder.This case study indicates that hypereosinophilic syndrome should be considered a key differential diagnosis when initial skin symptoms arise.This article introduces a rare disease that has a variety of skin manifestations, including necrotic lesions and cutaneous vasculitis on the back of the neck, which have been misleading, but other clinical and paraclinical evidence has been in favor of this rare syndrome, and in the treatment, drugs beyond hypereosinophilic therapy have been required to achieve improvement.
