ABSTRACT Radial collateral ligament (RCL) injuries of the thumb metacarpophalangeal (MCP) joint are relatively rare compared to ulnar collateral ligament lesions. These injuries typically occur due to forced and sudden adduction of the thumb and can significantly impair MCP joint functionality. In chronic cases, direct ligament repair is often unfeasible, necessitating various reconstruction techniques, including the use of the abductor pollicis brevis (APB) tendon. This manuscript aims to review the literature on APB‐based reconstruction techniques and to present two cases of chronic RCL tears treated with a modified reconstruction technique using a dorsal half‐slip of the APB tendon, along with their functional outcomes.
