Maternal mirror syndrome associated with fetal sacrococcygeal teratoma (SCT) presents considerable maternal morbidity and high fetal mortality. This report presents and discusses a case of rapidly enlarging SCT complicated by maternal mirror syndrome. Serial ultrasound examinations identified an expanding and highly vascularized tumor, further complicated by fetal hydrops. The onset of maternal mirror syndrome indicated the severity of fetal‐placental compromise, ultimately necessitating an emergency preterm delivery via cesarean section. The postnatal course was further complicated by severe internal tumor hemorrhage, which was unresponsive to resuscitative efforts. Following delivery, maternal edema showed gradual resolution over 5 days, and the patient was discharged in good condition on day seven.
