Abstract Spontaneous appendicocutaneous fistula is an extremely uncommon complication of acute appendicitis. We report a 6-year-old boy who developed a persistent feculent discharge from the right flank over the last 2 years after an inadequately treated case of appendicitis. Communication with the cecum was confirmed by diagnostic evaluation with fistulography. Appendectomy with complete excision of the fistulous tract resulted in full recovery.
