This article provides an updated management algorithm for dermatophyte onychomycosis (OM), aimed at standardizing diagnostic and therapeutic decision making. OM is a common condition in dermatology and primary care settings. Nail treatment should only be initiated after mycological confirmation, as approximately half of nail dystrophies are attributable to fungal infection. Unlike inflammatory nail disorders, dermatophyte OM is quite easily curable when managed appropriately. Accurate diagnosis, appropriate treatment selection, patient adherence, and consistent clinical follow-up are key to successful outcomes.
