Objectives: To present atypical clinical and demographic presentations of oral mucosal melanoma and highlight diagnostic and therapeutic challenges. Methods: Two clinically unusual cases of oral mucosal melanoma were evaluated with clinical examination, imaging, histopathology, and immunohistochemistry. Multimodal treatment approaches were documented along with follow-up outcomes. Results: The first case involving a middle-aged female with widespread metastasis showed poor response to chemotherapy and unfavorable outcome. The second case involving a young male demonstrated successful disease control following surgical resection combined with proton therapy and immunotherapy. Conclusions: Oral mucosal melanoma may present at non-classical sites and in younger individuals. Early biopsy and multimodal treatment strategies significantly influence prognosis and survival outcomes.
