ABSTRACT Invasive pulmonary aspergillosis can occur in patients without classical immunosuppression, particularly those with COPD and corticosteroid exposure. Early recognition, prompt antifungal therapy, and vigilance for neurological symptoms are essential to prevent central nervous system dissemination, which carries high mortality and significant long‐term neurological morbidity despite aggressive treatment.
