Cemento-osseous dysplasia (COD) is a benign fibro-osseous lesion that can present challenges during periodontal regenerative surgery owing to altered bone architecture. This case series reports on three patients with COD who underwent periodontal regenerative/reconstructive surgery, demonstrating significant clinical and radiographic improvements. Despite COD's potential impact on healing dynamics, successful regeneration was achieved through careful surgical planning and the use of regenerative materials. These findings suggest that COD does not necessarily preclude successful periodontal regeneration and highlights the importance of individualized treatment approaches. Further research is needed to optimize regenerative strategies in patients with COD and understand long-term outcomes. CPD/Clinical Relevance: Successful periodontal regenerative surgery in patients with cemento-osseous dysplasia is possible, despite the challenges posed by the lesion.
