A bstract Pleomorphic adenoma tends to recur from poor surgical techniques or malignant ex-pleomorphic, resulting in high morbidity. There was a 41-year-old man who had an excision of the right parotid tumor at a university health center, but it recurred after the fourth year, which necessitated a repeat superficial parotidectomy in our center. Although, he noticed the recurrence again 3 years after the second surgery delayed for another 3 years before reporting. Unfortunately, he had an additional 2-year delay because he objected to going to the referred facility with a nerve stimulator (NS). At presentation, the mass measured 9 cm × 13 cm, with facial disfigurement, but no facial nerve involvement. Magnetic resonance imaging showed the facial nerve branches embedded with the tumor. Subsequently, he had total parotidectomy assisted by a NS newly acquired by our facility. The surgery was uneventful and histology confirmed a pleomorphic adenoma. The patient is currently being followed up. A NS aids in the complete excision of recurrent pleomorphic adenoma and reduces iatrogenic injury to the facial nerve.
