Traditional audiological assessments such as pure-tone audiometry and basic speech recognition tests provide limited insight into the real-world listening challenges faced by paediatric cochlear implant (CI) users. Functional hearing outcomes, including speech perception in noise, auditory localization, and effective communication, are increasingly recognized as critical indicators of success. This narrative review examines functional hearing abilities in children with cochlear implants, emphasizing the importance of binaural hearing and advanced signal processing technologies. Evidence from existing literature and clinical observations indicates that bilateral auditory input significantly enhances spatial hearing and speech perception in complex acoustic environments. Additionally, modern cochlear implant processors equipped with adaptive signal processing algorithms further optimize listening performance. The findings underscore the need to move beyond traditional audiograms and adopt a comprehensive, functional approach to auditory assessment and rehabilitation.
