68 Ga-labeled somatostatin receptor (SSTR) antagonists such as 68 Ga-NODAGA-JR11 are increasingly used for imaging neuroendocrine tumors (NETs) owing to their higher lesion detection rates compared with agonists. Incidental findings on these scans can occasionally provide unexpected insights. We report a patient with NET who demonstrated diffuse uptake of 68 Ga-NODAGA-JR11 in the left ventricular myocardium. This unusual finding raises considerations regarding non-tumor binding sites of SSTR antagonists and their potential physiological or pathologic correlates. Awareness of such uptake patterns is essential to avoid misinterpretation.
