This report provides an independent editorial and technical evaluation of the preprint "MARCUS: An agentic, multimodal vision-language model for cardiac diagnosis and management" (arXiv:2603.22179). The assessment focuses on the system's multimodal architecture, clinical reasoning capabilities, and methodological rigor. While acknowledging the technical sophistication of the MARCUS framework, this review identifies critical gaps regarding real-world diagnostic uncertainty and the current reliance on structured benchmark evaluations over clinical validation.
