Background: Antiretroviral therapy (ART) has improved the quality of life of women living with HIV (WLHIV), increasing their intention and attempts at having children. Educating WLHIV on safer conception strategies helps them to navigate the periconception HIV transmission risk. However, understanding the context-specific needs of WLHIV is crucial in tailoring the provision of SC services. Nonetheless, this is a challenge in Ghana due to information dearth on the subject. Aim: To explore the safer conception needs of WLHIV who intend to have biological children in the Oti and Volta Regions of Ghana. Methods: In a qualitative exploratory design, in-depth interviews were conducted with 24 purposively sampled WLHIV who intended to have children within 24 months. Braun and Clarke’s thematic analysis was employed in data analysis. Results: The needs identified by WLHIV include the need for healthcare worker-initiated communication, education on SC strategies, couple-based education on HIV prevention, continual ARV supply, assistance with fertility, and empathy. Conclusion: WLHIV have safe conception needs. This finding should inform the organisation of HIV reproductive health and nursing care to ensure a WLHIV-centred service that can facilitate the uptake of SC, thereby contributing to the prevention of periconception HIV transmission.
