Simultaneous bilateral spontaneous pneumothorax (SBSP) is an extremely rare phenomenon. A case involving a young male with asthma and SBSP is reported. Chest computed tomography revealed right apical bulla and re-expansion pulmonary edema. Left lung collapse and pneumonia were found. After treatment, his respiratory condition was stabilized; however, air leak in the right lung persisted. The patient underwent thoracoscopic right lung cystectomy and was discharged. Elective surgery should be performed as soon as possible.
