Abstract The challenge of managing massive irreparable rotator cuff tears is not solved through technical complexity alone but through achieving balance between biological restoration and mechanical reinforcement. Superior capsule reconstruction using the long head of the biceps tendon offers static and dynamic advantages, whereas tendon transfers provide additional posterior force vectors to restore external rotation. The combined “static–dynamic” strategy may represent an evolving biologically based alternative for selected patients who are not candidates for arthroplasty.
