Esthesioneuroblastoma (ENB) is a rare malignant neuroectodermal tumor of the nasal cavity. Metastatic spread is mainly to the cervical lymph nodes, lungs and bone. The grading of ENB is extremely important as individualized treatment plans must be formulated according to tumor grade. We report the case of a 28‐year‐old female who presented with cervical adenopathy, headaches and several episodes of epistaxis over the summer of 2023. Radiological imaging was suggestive of a voluminous heterogeneous nasosinusal tissue lesion centered on the right ethmoid, olfactory cleft and right nasal cavity, with invasion of the base of the skull (osteolysis) and basifrontal endocranial extension, nerve invasion (right V2 branch and right vagus nerve), extension to the extra‐conical fat of the right orbit, associated with multiple bilateral cervical and right retropharyngeal adenomegaly. A biopsy of the mass confirmed an esthesioneuroblastoma and the pathological examination revealed a Hyams Grade IV. Positron emission tomography (PET) coupled with a CT scan did not reveal any distant metastases. Chemotherapy was received, and after evaluation and the good local response, a concomitant radiochemotherapy was then received . We report a huge ENB with cervical lymph node metastases. Magnetic resonance image and computed tomography are helpful for evaluating the appearance and the extent of ENB, as well as grading this tumor, which may aid therapeutic decisions and improve survival.
