ABSTRACT Bladder outlet obstruction secondary to an obstructing prostatic cyst is a rare clinical entity. Incomplete regression of the Müllerian duct is supposed to be the cause of such cysts. Clinical features can range from acute bladder outlet obstruction to chronic urinary retention, oligospermia, and prostatitis‐like symptoms. Diagnosis is challenging and requires ultrasonography of the abdomen and pelvis, transrectal ultrasound, and even magnetic resonance imaging. Transurethral resection or unroofing of a prostatic midline cyst near the urethra has been a safe and effective method of managing prostatic cysts. This article describes a rare case of bladder outlet obstruction caused by a prostatic midline cyst without an enlarged prostate. The patient was treated with urethral catheterization, diagnostic cystoscopy, and cyst deroofing. This case adds valuable insights to the literature.
