Abstract Mycobacterium haemophilum infection typically presents with cutaneous manifestations. Absence of cutaneous involvement is rare and diagnostically challenging. We report two immunocompromised adults, a liver transplant recipient and a patient receiving tumor necrosis factor (TNF) α inhibitor, without cutaneous manifestations. These cases expand the recognized clinical spectrum and underscore the importance of considering this pathogen in immunocompromised hosts even in absence of cutaneous involvement.
