Abstract：Under the backdrop of hospital reform and high-quality development，the group development of hospitals faces numerous challenges. The demand for medical services from society and patients continues to rise，and the competition among hospitals has become increasingly fierce. People's expectations for convenient and efficient medical services are also growing day by day. Nowadays，the comprehensive level of hospitals is no longer solely determined by hardware facilities; the soft power of service quality has become increasingly crucial. The post-discharge follow-up of patients，as an important continuation of the medical process，lies in achieving full-cycle health management，which is of great significance for patients' recovery and the improvement of medical quality. Taking patient follow-up as the entry point，promoting public medical institutions to transform from extensive management to refined management is a key link in the group development of hospitals，and it can better highlight the core content of medical institution's efficiency management under the new pathway.
