Papillary renal neoplasm with reverse polarity (PRNRP) is very rare. Herein, we report a case of imaging findings of PRNRP in a 75-year-old woman. On the 68 Ga-FAPI-04 PET/CT and 18 F-FDG PET/CT image, it presented as a soft-tissue nodule in the left renal pelvis with intense FDG uptake (SUVmax: 20.8) but no FAPI uptake. Postsurgical pathology supported a diagnosis of PRNRP.
