This study aimed to measure the 9-month clinical results of different cavity patterns and treatment methods for first permanent molars (FPM) with molar-incisor hypomineralization (MIH). Fifty patients, aged 7–14 years, were included in this study. Eighty molars with MIH, meeting the study criteria, were restored using four different treatment approaches: Invasive treatment group, Invasive treatment + 5% NaOCl group, Non-invasive treatment group, and Non-invasive treatment + 5% NaOCl group. Restorations were measured at 1- 6- and 9-month intervals using the modified United States Public Health Service (USPHS) criteria. The median age of the children was nine years. Upon clinical examination, the median ICDAS-II score was 6. For the primary dentition, the median dmft score was 3, and in the permanent dentition scores were 3, and, 4 respectively. No statistically significant differences were found between the groups regarding anatomical form, marginal adaptation, marginal discoloration, secondary caries, postoperative sensitivity, and retention during the follow-up periods. Moreover, in terms of the surface texture of the non-invasive treatment group, there was a statistically significant difference between at the 1st and 9th months. Characterization of cavity patterns and selection of appropriate treatment modalities for molars affected by MIH are crucial in pediatric dentistry and should be thoroughly evaluated by pediatric dentists to achieve optimal patient outcomes.
