Risk stratification and management of anomalous coronary arteries remain controversial. Interarterial anomalous aortic origin of the left coronary artery is generally considered to be an indication for surgical repair. We present 6 cases of unrepaired anomalous left coronary artery with an interarterial course but without other high-risk features in pediatric athletes evaluated in our coronary anomalies management program to highlight specific anatomical considerations in this diagnosis and their implications for risk stratification and management.
