Abstract Prodromal dementia with Lewy bodies (DLB) refers to the phase preceding the onset of dementia, characterized by clinical signs or symptoms that suggest the potential development of DLB. Prodromal symptoms may include cognitive deficits, sleep disorders, autonomic dysfunction, and neuropsychiatric disturbances. Three prodromal DLB syndromes have been proposed: mild cognitive impairment (MCI) onset, delirium-onset, and psychiatric-onset presentations. This case report discusses a 67-year-old female with MCI along with a history of thought and perceptual disturbances. It highlights the criteria for diagnosis of prodromal DLB, as early diagnosis may improve clinical outcomes through early interventions.
