Introduction Multiple myeloma is an incurable hematologic cancer. Some patients with this condition develop relapsed or refractory multiple myeloma and are resistant to current standard-of-care options. Belantamab mafodotin (belamaf; BLENREP, GlaxoSmithKline) is a first-in-class antibody-drug conjugate that targets B-cell maturation antigen used to treat relapsed or refractory multiple myeloma. Corneal pseudomicrocysts are the most common side effect of this medication. This case describes corneal pseudomicrocysts in a patient receiving belantamab mafodotin therapy. Case Report A 53-year-old African American woman presented for anterior segment evaluation prior to initiation of belantamab mafodotin therapy to treat relapsed or refractory multiple myeloma. Baseline examination findings showed a clear cornea. Fifty days after the initial infusion, she developed corneal pseudomicrocysts that led to a decrease in vision in 1 eye after 71 days. After treatment was discontinued, the cornea and vision returned to baseline. Conclusion Antibody-drug conjugates represent a promising development in the treatment of refractory multiple myeloma, but these medications can cause corneal pathology. Optometrists should be aware of ocular toxicity which can be associated with these medications and should work closely with hematologists and oncologists who prescribe these medications to ensure that patients who are treated with these medications maintain optimal vision and ocular health.
