Abstract Midline forehead swellings represent a diverse and clinically challenging group of pathologies ranging from benign congenital masses to life-threatening infections. These swellings are of significant clinical importance due to their potential for intracranial extension and communication with the central nervous system. The clinical presentation varies significantly depending on the underlying pathology and the age of the patient. Imaging includes ultrasound, computed tomography scan, and magnetic resonance imaging; however, advanced imaging is mandatory for surgical planning. This review highlights the need for a high index of suspicion, a systematic clinical history, comprehensive radiological imaging, and a multidisciplinary approach for favorable cosmetic and functional outcomes.
