Basal cell carcinoma (BCC) is the most common skin cancer, often requiring excision and reconstruction for facial defects. We report a 72-year-old male with recurrent BCC on the left nasal sidewall and medial canthus (2 cm × 2 cm ulcer). Wide local excision with 5 mm margins was performed under general anaesthesia, followed by cheek advancement flap reconstruction. The flap was mobilized along the infraorbital margin and nasolabial fold, ensuring perfusion before layered closure. Postoperative follow-up at 3 months showed no recurrence, flap loss, ectropion, or functional impairment, with minimal scarring and excellent aesthetics. Cheek advancement flap is reliable for medium-sized nasal defects due to its laxity, wide pedicle vascularity, and low complication rate.
