Abstract This technical note describes using knotless femoral fixation for medial patellofemoral ligament reconstruction for skeletally immature patients. Using a knotless anchor for epiphyseal femoral fixation in the skeletally immature decreases the risk of irritable interference screw fixation and optimizes graft contact with host bone. Optimal tunnel position and depth, graft delivery, and secure anchor fixation are the keys to a successful reconstruction.
